Booking Policy: All bookings may be subject to a minimum of a $50 deposit at the time of booking. Clients will be asked to fill out health, safety, & or liability waiver(s) on the appointment day or prior to their appointment.
Cancellation Policy: Deposits are nonrefundable. If rescheduled 24 hours prior to the appointment, deposits may be applied to the rebooking. If an appointment is canceled less than 24 hours in advance, the client forfeits the right to a refund of the initial deposit. Clients are given a 10-minute grace period. Upon our arrival, If late beyond 10 minutes, the appointment will be canceled and the patient forfeits the right to a refund of the initial deposit.
Medical History Form and Medical Evaluation: All patients are required to complete a medical history form when booking an appointment. A medical evaluation will be completed at the start of every appointment by a licensed medical professional. Clients should alert the staff and refrain from receiving services if they have been told to avoid our services by a medical professional, or if they have any of the following conditions: open cuts, abrasions, sores, infections, illness, or the inability to maintain personal hygiene, or if they have any condition that poses a threat to their health and/or safety or the health and/or safety of others. In addition, all clients agree to receive services in accordance with all public health requirements. All patients will be assessed to determine candidacy to undergo all Aesthetics, Weight Loss, & Wellness services. It is the responsibility of each client to consult with their medical professional to determine if they have any of the above medical or other conditions and whether such a condition poses a threat to anyone’s health and/or safety.
We reserve the right to deny entry or services to any client over concerns for health and/or safety risks.
Confidentiality: We at Figurations comply with HIPAA guidelines and maintain confidentiality. Information regarding the specific nature of your treatment is maintained at Figurations and is considered confidential within the office unless specified in writing. However, each provider reserves the right to use specialty consultation with other medical providers as deemed necessary.
